St. Ethelreda admits students of any race, religious affiliation, color, sex, national and ethic origin to all rights, privileges, programs and activities generally accorded or made available to students in attendance.

1. Student Registration Form

The Student Registration Form is used when registering for a new school year. This form was updated May 2011.

When registering for preschool, please use the Eagle's Nest Registration Form.

4. Field Trip Permissions Form

The Field Trip Permission Form will typically be completed by the school and distributed to students. A blank Field Trip Permission form is included here "just in case".
